The Hospice of the Good Shepherd Team would like you to join them to light up a life and remember those people who are sadly no longer with you to share special times like Christmas. During December 2011, Christmas Trees and the Hospice will be illuminated by hundres of lights, each representing the warmth of those special to you and helping to keep their memory alive.

We will commemorate your loved one(s) by sending you a specially designed card with their name(s) and a Memory Satr to hang on the Hospice Chrsitmas Trees or on your Christmas Tree at home, which can include a personal message or photograph. Their name(s) will also be included in the Book of Life.

One star will be sent per dedication form (additional stars available from the Hospice)

Every name will appear in the Book of Life unless otherwise stated, however we do need to receive you donation and dedication name by Friday 2nd December if you wish the name of your loved one to appear in the Book in time for our Light up a Life service in Backford. We are not able to enter names in the Book of Life after 2nd December.
